Rapha Healthcare Limited is a Semi-Independent Supported Living Organisation located within London and has homes within the immediate surroundings areas.

Rapha Healthcare help to transform lives of vulnerable children, young people and adults who are at risk of social exclusion through our residential services by taking a holistic approach and having a focus on the core values of being non-judgmental, providing high standards of support and compassion.

Each individual is accepted as a unique person, and we work with them so they can feel safe, secure and inspired to live fulfilling and independent lives. It is our believe that in order to deliver an excellent service, individual’s needs should be at the heart our business and everything we do.

The type of support we offer varies depending on the young person. However, no matter what the individual needs are, staff will always support young people to work towards their goals and build their confidence and independence for the future.
Rapha Healthcare is working in close partnership with West London Alliance to ensure a high standard of support is achieved in our homes.

Our current placement contract is with Hammersmith and Fulham Council, however, West London Alliance will oversee all aspects of this service.

Aims and Objectives

Rapha Healthcare aim is to provide accommodation and support for 18+ years old to prevent them from becoming homeless, socially excluded and be able to maintain and live independently in the community. Our staff will assess the needs of the individual person and assist them to build the skills required to live independently ensuring a positive quality of life.

PRINCIPLES:

To enable us to achieve the highest standards of accommodation and support possible we will:

  • Provide a home environment, that young person can be proud of and feel able to invite family and friend
  • Work in collaboration with colleagues and other agencies to build strong relationships with support young person and their families.
  • Provide a homely environment in which young person will be encouraged to develop physically, psychologically, intellectually and socially.
  • When appropriate, offer specialist support and counsel from experienced staff who will encourage and enable young person to achieve independent living. We will make external resources accessible to young person when considered required by their individual pathway plans.
  • Provide a safe home environment where the young person can reflect on their experiences, stabilise their behaviour, regain control and find coping mechanisms.
  • Promote the independence of the young person at all times and encourage them to care for themselves by offering a high level of emotional support and practical help in
    health and educational/employment issues, cooking, budgeting and household tasks.

OUR SERVICES

Purpose of the service
Our purpose is to provide a high standard of accommodation and supportive environments for young people on their path to independence. The service we offer ensure that the young person is able to develop and maintain respect, self-reliance and self-care skills and facilitate them to make informed, age-appropriate decisions, concerning their present and future so they can achieve a successful transition into independence and adulthood.

All young people will receive support with:
• Personal Hygiene routines
• Keeping a clean and safe living environment
• Food preparation and healthy eating
• Food and grocery shopping and budgeting
• Joining and attending leisure and health activities
• Accessing and registering with health services
• Healthy relationships and safe sexual health
• Education, training or employment
• Family contact
• Taking care of their emotional and mental health
• Religious and cultural needs/preferences
• Reducing substance and alcohol abuse
• Safety and self-harm reduction
• Managing emergencies
• Socialising and community integration
• Bill payments, banking, savings and money management
• Obtaining independent accommodation and accessing a tenancy
• Joining the housing register and support with bidding for council properties

OPERATING HOURS
Rapha Healthcare provides a 24/7 Semi-Independent service. Our flexible working hours will ensure staff Tare able to offer effective support to all the young people throughout the day and night.
