Returning home from a hospital stay, following an acute illness, injury or surgical procedure can be life changing to the mind, body and spirit. Some people are faced with many new and unexpected challenges when discharged home. For instance, they may struggle to obtain the physical help and home support they need while they recuperate.

Our Management Team, Care Co-ordinators and Field Care Supervisors are all Registered Nurses or Qualified Health Care Assistants with experience, compassion and attentive will ensure you or your loved one enjoys a complete and speedily recovery at home. We work with individuals and family, Hospitals, General Practitioners, Community Nurses, and other bodies to make sure that the person receives quality and the best possible care package for optimising recovery and getting each person quickly back to the most independent lifestyle possible. We can create a tailored care plan that meets the parent’s unique needs with one or more of the following services:

• Changing dressings
• Monitoring medications, and drug pickups description
• Change in condition alerts
• Arranging transportation and scheduling of doctors’ appointments
• Providing basic daily activities like bathing dressing and personal care
• Assistance with mobility.

We also offer non-medical home care services including meal preparation, shopping, cleaning, transportation, Assistance with daily activities and some specific household chores.
Our home care staff have good knowledge and experience in helping solve the new and unexpected challenges of the patient transitions from an acute care environment to home.
