Rapha Healthcare Limited (RHL) is committed to helping young people leaving local authority care make a successful transition to independent living and education or employment.

OUR VISION

  • To provide a range of high-quality support services to Look after Children who are Leaving Care, supporting young people to learn, develop, work, and reach their full potential.

  • To be directly involved in the design and delivery of support services and ensure that young people's needs are met.

  • To provide young people support with a home for life – a home where they can live as independently as possible in a safe and supportive environment.

 

NATURE OF OUR SERVICE

RHL provides comprehensive services offering support and accommodation in the London Boroughs of Hillingdon and surrounding Boroughs to care leavers aged between 18 and 25 years old of both sexes. The house is of a high quality, and is located in a residential street. It is also located within areas that are close to transport, colleges, universities, and other amenities.

Our services are tailored to everyone’s needs. We do not provide nursing care. Our services and environments can be adapted to suit each young person’s needs.

Young People will be involved in the decision-making process of how the service is delivered, including all daily routines within the house. Each person is allowed to make their needs known, and those needs are identified and acted upon as part of their Individual Support Plan.

The service will be provided for any length of time required. There are no limits set, ensuring that services can be bespoke and appropriate care plans can be put in place with realistic time scales.

This service does not provide nursing care.

Levels of Services and Support

The passion and commitment we have for our young people always require us to provide the highest standards of support and to consistently encourage the development of opportunities for our young people, and our skilled, dedicated staff team. Our supported living service is managed by a team of professionals with a lot of experience in this field.

The young people are equipped with all the skills they require and supported to move into permanent independent living, making a smooth and successful transition into adult life.

The service ranges from:

  • Shared accommodation

  • Stand-alone accommodation

  • Outreach work

  • Emergency 24hr referrals

When a young person is placed with us, we work with the young person to develop a routine that enhances their quality of life. Examples of the basic tasks we will monitor and support the young people with are:

  • Registering with the GP, dentist, and optician
  • Support with life skills to promote independence.
  • Support with money management (e.g. budgeting, saving, managing debt, accessing bank accounts);
  • Support with accessing benefit entitlements;
  • Support with managing future tenancy and the rights of tenants;
  • Support with managing household and all aspects of daily living (e.g. paying bills. cleaning and other practical skills);
  • Support with shopping, cooking and eating a healthy diet;
  • Support with personal hygiene;
  • How to access support/help from the community;
  • Support in managing and attending appointments in the community and with other professionals;
  • Support to access education, employment, or training;
  • Support with making informed choices in their lives;
  • Support with accessing information about leisure opportunities available to them in the local area;
  • Support in learning how to keep themselves safe in the community and their own home

 

RHL is experienced in working with young people as well as those who have extremely challenging and risky behaviour. The support plan will reflect the needs of the young persons and will offer the following;

  • Support to development of reflective thinking
  • Offending reduction programmes in partnership with YOT and other agencies
  • Support with reducing substance or alcohol abuse
  • Work around young people suffering from post-traumatic stress disorder
  • Support with asylum applications and other legal matters
  • Support for young parents (Outreach support)
  • Gang awareness and intervention
  • Support with reducing risky behaviour that could lead to exploitation
  • Support with managing ADHD, including anger management, and temper control disorder

Through our well-structured risk Assessments, we aim to ensure that independence and fulfillment are maximised for the young people we support.

Accommodation/Facilities:

This home is fully modernised and completely refurbished, with fully equipped spacious bedrooms that can be decorated to the bespoke needs of each individual, and well-appointed communal areas. There is one shared bathroom upstairs and one shower room downstairs for young people. There are also two ensuite bedrooms. All properties further benefit from state-of-the-art CCTV systems in all outside and communal areas to ensure the safety of all our young people.

Part of the garden is used for young people to grow their own vegetables, especially in summer.

Each young person is provided with a private bedroom that has sufficient space to accommodate a reasonable number of personal possessions. The bedrooms are equipped to meet the young person’s individual needs and have sufficient sockets and internet connectivity to enable young people to maintain relationships with family and friends and feel connected.

Young people are provided with appropriate furniture, such as desks and chairs in their rooms for private studies etc.

The Young people are provided with a welcome pack which has ensure that basic items such as bedding, towels, personal hygiene products, toothpaste and brush, cooking oil, rice, tin tomatoes, pasta, milk, cereal, and bread.

The kitchen is equipped with cooking utensils, a cooker and an oven, a fridge freezer, a washing machine, and cleaning products to use when needed. There is a dining area for young people to sit and enjoy their meals.

RHL ensures that young people have consistent and continuous access to local services, including education, healthcare, fitness clubs, and religious activities.

The house is located close to a local park that has football and basketball pitches, and an open gym that enables young people to actively participate in physical activities and form relationships in the local community. This promotes positive self-esteem and a sense of belonging within the local community.

A location assessment was carried out initially before opening the home to ensure that the location of the premises used as supported accommodation is suitable for young people.

OPERATING HOURS
Rapha Healthcare provides a 24/7 service. Our flexible working hours will ensure staff are able to offer effective support to all the young people throughout the day and night.
